Pinnacle CSI Competitive Benefit Package

Pinnacle CSI puts employees first. From flexible work schedules and performance-based incentive compensation, to one of the most competitive benefit packages in the business, we are committed to providing a supportive work environment and a secure future for our people and their families.

In addition to paid vacation, holidays, and sick leave, all regular full-time employees are eligible for Pinnacle CSI's health care plans, retirement program, flexible spending program, disability income, survivor protection, military leave, and tuition reimbursement.

Health Care Plans

Pinnacle CSI offers 100% paid (premiums) medical, vision, and dental insurance coverage to employees and their eligible dependents.

Medical & Vision
Pinnacle CSI employees have health and vision coverage through Anthem Blue Cross and Blue Shield.

Anthem Keycare: This plan is a no-copayment, high-deductible plan that covers 100% of most basic medical services, from doctor visits to lab tests, after a $1,200 annual deductible ($2,400 for family) has been met for any health care provider. Pinnacle CSI sets aside $750 annually for employees ($1,500 for family) into a separate HRA (Health Reimbursement Account), of which unused amounts roll-over into subsequent years.

Anthem Blue View Vision: Our plan provides preventive eye care, including examinations and corrective lenses, through the Blue View Vision Network. Discounted laser vision correction surgery also is available through the network. Annual in-network exams are provided at no charge after a co-payment. Services outside the network are reimbursed according to a pre-set allowance schedule.

Dental
Pinnacle CSI's Preferred Dentist Program through MetLife pays 50%, 80% or 100% of preventive, basic and major dental care costs depending upon the treatment involved up to an annual per-person maximum of $1,500. Basic and major PDP dental services require an individual and family annual deductible. Our plan reimburses the same percentages for non-network providers' reasonable and customary charges, which generally are higher than the pre-negotiated fees of participating PDP providers.

401(k) Retirement Plan

Pinnacle CSI's 401(k) Program helps employees secure their future with employee pre-tax contributions, company matches, and profit sharing plan contributions. Employees are automatically enrolled in the Pinnacle CSI 401(k) plan immediately following employment.

Pinnacle CSI matches 50% of the first 6% employee contribution and 3% of an employee's eligible compensation under the plan's "safe harbor" provisions. Employee and "safe harbor" contributions are immediately vested.

As part of the company's profit sharing plan, Pinnacle CSI contributes a percentage of adjusted profits before taxes to each employee's 401(k) based on the individual's eligible pay. Enrolled employees receive profit sharing and "safe harbor" contributions whether or not they make contributions to their 401(k) plan.

Leave and Holidays

Pinnacle CSI offers vacation based on the employee's years of service, ranging from two to five weeks. Pinnacle CSI also observes 10 federal holidays and allows for six of them to be ‘floating.’

Flex Spending Program

The Pinnacle CSI Flex Spending Program gives employees the option to pay for certain out-of-pocket health care and dependent child/elder care expenses on a pre-tax basis. Employees can elect to participate in one or both accounts.

Health Care Flex Spending Program
Pinnacle CSI reimburses up to $5,000 per year supplemental expenses considered eligible by the IRS and not paid under the group medical, dental or vision care plans. These expenses include insurance deductibles, amounts exceeding reasonable and customary costs, hearing aids, prescribed contraceptives, and orthodontia.

Dependent Care Flex Spending Program
Pinnacle CSI reimburses up to $5,000 per year to cover care for an employee's child up to age 13 or other dependent who is not self-sufficient and lives in the home at least eight hours a day and is claimed as a dependent on the employee's federal tax return.

Disability Income

Pinnacle CSI provides income continuation insurance at no cost to all regular full-time employees in the event that illness or injury prevents the employee from working through Short-Term Disability (STD) and Long-Term Disability (LTD) insurance plans. In addition, Pinnacle CSI pays the base cost of a Long-Term Care plan for employees.

Short-Term Disability
STD provides 60% of the employee's total compensation up to $2,000 per week.

Long-Term Disability
LTD coverage pays 60% of the employee's total compensation up to $10,000 per month.

Long-Term Care
The Long-Term Care base plan provides $4,000 per month for a long-term care facility and $2,000 per month for home care. Employees can add dependents at their cost and at group rates.

Survivor Protection

Pinnacle CSI pays the first $50,000 of an employee's Life and Accidental Death and Dismemberment (AD&D) Insurance coverage to assist families financially in case the employee dies or is severely injured. Employees can increase Voluntary Life and AD&D benefits at their cost and at group rates.

Tuition Reimbursement

Pinnacle CSI supports continuing education for employees, providing up to $5,250 annually for tuition, books, laboratory fees, and course materials for approved continuing education courses.

Business Travel Insurance

Employees receive up to $250,000 of company-paid insurance coverage while traveling on business for Pinnacle CSI.

Military Leave

At Pinnacle, we value the contributions and sacrifices our employees make when they are asked or volunteer to serve our Country as members of the uniformed services. Pinnacle pays eligible employees on approved Military Leave the difference between military full duty pay and their Pinnacle base salary for 6 months or until the termination of their military service, which ever comes first. As permitted by the applicable health insurance carrier, Pinnacle continues and covers the cost of the medical, dental and vision care coverage in effect for employees and/or their dependents, and the employee's life insurance and AD&D coverage of $50,000 for the duration of Military Leave.
